Policies & Fees
Cost
All sessions are $160. For out-of-network clients, I am happy to provide superbills for those with policies that accept them.
I am currently in-network with Aetna health insurance. If you would like to use health insurance, please let me know when you contact me.
Booking
To request a first appointment, either reach out via the Contact page, email katelynclevelandmft@gmail.com, or call the office at (650) 394-6589.
After the first appointment, I will work with you directly to schedule a regular/repeating appointment to reserve the time slot for you going forward.
Payment
Clients are required to have a credit/debit card on file. Payments are due on the day of service, processed through IVY Pay for private fee, or Headway for those with insurance.
Cancellations
There is no charge for cancellations made with at least 24-hour notice (email, text, or phone call is sufficient for notice). Cancellations with less than 24-hour notice will be charged the full fee for the missed session.
License Verification
It is a good idea to verify that your therapist is licensed, that license is up-to-date, and that the therapist is in good standing with the law. You can find a link to the licensing board verification sites for California here (DCA – BBS): https://search.dca.ca.gov/
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your
medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
